Thank you for taking an interest in engaging with services provided by Charlotte Hazeldine Chinese Medicine (‘CHCM’). Please read this document carefully. By signing the Patient Consent Form you or your authorised person(s) acknowledge that you understand and accept the following Terms and Conditions and all policies referred to herein.

If you do not agree, or cannot comply with any of the following Terms and Conditions, do not book a consultation and/or treatment and/or make payment and do not attempt to access the CHCM site.

Health & Wellbeing

Medical History

It is the patient’s (or authorised person’s) responsibility to provide CHCM with a comprehensive, correct, honest and true medical history. CHCM reserves the right to refuse treatment if CHCM believes the medical history provided is incomplete, inaccurate or incorrect. All patient information will be handled in accordance with CHCM’s Privacy Notice.

Medication

CHCM requires that the patient, or their authorised person, discloses any and all prescribed medication and any other supplements or vitamins being taken. CHCM reserves the right to decline treatment to a patient whose full medication list has not been disclosed. CHCM will not be held responsible for any adverse events arising from incorrect, incomplete or misleading information regarding medication.

Referrals

CHCM may, in certain circumstances, refer a patient to their GP and/or other healthcare professionals. If a patient refuses to contact their GP or other healthcare professionals after being requested to do so, CHCM reserves the right to decline further treatment and will not be held responsible for any consequences arising from this decision.

Interactions with Medication

Herbal medicine: CHCM currently does not prescribe or advise on herbal remedies or any other supplement or vitamin that may interact with a patient’s prescribed medication. As a consequence CHCM will not be held responsible for any consequences arising from the patient’s use of any herbs, supplements and/or vitamins.

Acupuncture: There are certain interactions that may arise from certain prescribed medications, for example blood thinners may increase the risk of bruising and bleeding. CHCM will inform the patient if CHCM believes that their medication may cause an adverse reaction to the treatment(s) provided.

Patient Conduct

The patient must conduct themselves appropriately and with respect to CHCM and other patients. Behaviour which is considered inappropriate, violent, abusive or sexual in nature will not be tolerated and will immediately result in the cessation of treatment without compensation to the patient. The patient may be reported to the police.

Appointments

Booking

By making a booking, the patient, or their authorised person, confirms that they understand and accept these Terms and Conditions and the policies referred to herein. A booking is not confirmed until payment has been received by CHCM. CHCM reserves the right to cancel a booking at any time.

Attendance

If a patient arrives significantly late to an appointment, CHCM reserves the right to cease the appointment without refund.

If a patient fails to arrive at an appointment, or cancels an appointment with less than 48 hours’ notice, CHCM reserves the right to charge a cancellation fee in accordance with these Terms and Conditions.

Video Consultations

Video Consultations are conducted via an agreed video conferencing platform. The patient must ensure that they have an adequate internet connection and that the video conferencing platform is accessible to them prior to the start of the Video Consultation. CHCM will not be held responsible for any technical issues arising from the patient’s internet connection or the patient’s inability to access the video conferencing platform.

Cancellations and Refunds

Cancellations by CHCM

CHCM reserves the right to cancel appointments at any time. CHCM will provide as much notice as possible. In the event of a cancellation by CHCM, the patient will be offered a full refund or the opportunity to reschedule the appointment.

Cancellations by the Patient

If a patient needs to cancel an appointment, they must notify CHCM by email at acupuncture@charlottehazeldine.com as soon as possible and at least 48 hours prior to the appointment.

Cancellations made with more than 48 hours’ notice: CHCM will offer the patient a full refund or the opportunity to reschedule the appointment.

Cancellations made with less than 48 hours’ notice: CHCM reserves the right to charge a cancellation fee of 50% of the appointment price.

No-shows: If a patient fails to attend an appointment without notifying CHCM, CHCM reserves the right to charge a cancellation fee of 100% of the appointment price.

Unforeseen Circumstances: CHCM will consider cancellation fee waivers on a case-by-case basis. CHCM’s decision is final.

Refunds

Refunds will be processed within 14 days of the cancellation. Refunds will be made via the original payment method.

Payment

Payment is due in full at the time of the appointment. CHCM accepts bank transfer, card payments or payment links. CHCM does not accept cash payments.

Intellectual Property

All content on the CHCM website, including but not limited to text, images, graphics and logos, is the intellectual property of CHCM and is protected by copyright law. The patient must not reproduce, distribute or use any content from the CHCM website without the express written permission of CHCM.

Limitation of Liability

CHCM will not be held liable for any loss or damage arising from the patient’s use of the CHCM website or the services provided by CHCM. CHCM will not be held liable for any loss or damage arising from the patient’s reliance on any information provided by CHCM.

Governing Law

These Terms and Conditions are governed by the laws of England and Wales. Any disputes arising from these Terms and Conditions will be subject to the exclusive jurisdiction of the courts of England and Wales.

Changes to these Terms and Conditions

CHCM reserves the right to change these Terms and Conditions at any time. Any changes will be posted on the CHCM website. The patient’s continued use of the CHCM website or services following the posting of any changes will constitute acceptance of those changes.

Parking

Please note that the Chiltern Neuro Centre car park is reserved for Chiltern Neuro Centre members only. The clinic is embedded within a residential area. There is on-street parking outside the premises, but please be mindful of where you park and that you do not obstruct resident’s driveways or park in the residential bays. Please allow yourself enough time to find and appropriate parking.
